I was 38 when I found out why my hair was thinning

For years I put my thinning hair down to stress, to work, to getting older. I bought better shampoo. I took a supplement with a nice label. I told myself it was the bathroom lighting. I was in the hair business and I still did not see it.

Then a blood test showed I was anaemic. The reason I was anaemic was a gluten allergy. I was 38.

Why nobody caught it

Iron is absorbed in one narrow stretch of the small intestine — the proximal duodenum. A few inches of gut doing almost all of the work.

That is exactly the stretch gluten damages in people who cannot tolerate it. The tiny projections that create absorptive surface flatten out. So you can eat iron-rich food every day, take a supplement, do everything right, and still run low. It goes in and does not get picked up.

Iron-deficiency anaemia is the most common effect of coeliac disease outside the gut, and for many people it is the only sign. No stomach pain. Just tired.

In the UK the average time from first symptoms to diagnosis is about thirteen years. Most adults are diagnosed between 40 and 60. Two thirds to three quarters of people who have it do not know. At 38, I was early.

What I am not going to tell you

I could write this the usual way — cut gluten, fix your iron, get your hair back. It would sell more.

Iron stores do recover on a gluten-free diet, but slowly: six to twelve months for haemoglobin, up to two years for full stores. As for hair, there is no controlled trial showing a gluten-free diet regrows it. In one series of five patients the diet healed the gut completely and did nothing at all for the hair.

Correct your iron because anaemia is worth correcting. Not because I promised you a hairline.

If this sounds like you

Talk to your doctor, not a supplement company. Three things worth knowing:

  • Unexplained iron, B12 or folate deficiency is itself a reason to test for coeliac disease. Ask.
  • Keep eating gluten until you are tested. Cutting it first can give a false negative.
  • Stop biotin before bloodwork. It interferes with thyroid and other assays — a hair supplement can corrupt the test you need.

Where extensions fit

They are not a treatment. They fix nothing under the skin. What they do is give you density back while your body does its slow work, so you are not counting months in the mirror.
